Announcement for Medicare Beneficiaries regarding open enrollment

Medicare Beneficiaries
It’s not too late to get your flu shot! Flu season has yet to peak in North Carolina. A yearly flu shot is recommended for everyone 6 months of age or older, even if you've had the flu. You do not need to be a patient of ours to receive your flu shot here. The US has epidemics of flu each year between September and May. Even healthy people can get the flu. This season, protect yourself, and those around you, by getting a flu vaccine. Walk-ins welcome! 919-875-8150

Our Mission:

"Family Medical Associates of Raleigh is a Family Medicine practice that provides primary health care services to patients of all ages following a team-based approach. We partner with our patients to provide a medical home that is respectful, compassionate, accessible, and comprehensive.

Downtown Raleigh

We offer Same Day Appointments.
Call the office at 919-875-8150

Family Medical Associates of Raleigh has a physician available after hours (evenings, weekends, and holidays) as a service to our patients to address urgent and emergent health care needs. If you have a question or a health issue arises and you are thinking about going to an Urgent Care or the Emergency Room, please call our office first. However, after hour calls are not for routine medication refills. If you run out of your meds over a holiday or weekend, your pharmacy will usually give you 2-3 days of medication until our office is open again.

Medicare patients: How to choose or change your primary on MyMedicare.gov

Choosing a primary clinician on MyMedicare.gov does not change your Medicare benefits or which clinician you can see. It gives your provider access to more tools or services to help coordinate your care, especially when the clinician participates in an ACO or other Medicare alternative payment models.

This video outlines the step-by-step process on how to choose or change your primary on MyMedicare.gov.

Also, you can refer to the Beneficiary Fact Sheet.

Help us provide better service to you by notifying medicare that FMAR is your primary care clinician.

August is National Immunization Awareness Month!

Shots can prevent infectious diseases like measles, diphtheria, and rubella. But people in the U.S. still die from these and other vaccine-preventable diseases. It's important to know which shots you need and when to get them.

Are you unsure about what immunizations you or your children should be up to date on? Check out the Centers for Disease Control and Prevention website for age specific information about vaccines and immunizations.

October is Breast Cancer Awareness Month!

https://healthfinder.gov/NHO/OctoberToolkit.aspx

According to the American Cancer Society, 2012 will bring 226,870 new cases of invasive breast cancer in the United States. In addition, 63,000 new cases of carcinoma in situ (CIS) will be found (the earliest form of breast cancer). At this time, the chance of a woman having breast cancer is 1 in 8. Breast Cancer death rates are decreasing, probably as a result of finding cancer earlier and due to the accesibility of better treatment. Mammography is the best screening tool used today to find breast cancer.

Early Detection Saves Lives!

New Way to Access Your Health Information!

Family Medical Associates of Raleigh (FMAR) is excited to announce that you can now access your health information from your mobile device by downloading and using an approved APP called an Application Programming Interface or API for short.  An API is basically an electronic messenger that allows you to have easier access to your health information and to share health information securely among your different health care providers.

For an approved list of API apps that will allow you to access your information, please visit the Allscripts Application Store at https://store.allscripts.com/Partners/Details/FHIR%20Applications.  Once on the site you can click the Category menu and select “FHIR Apps” to view a list of Allscripts-tested APPs for your mobile device.  Then go to your mobile device’s App store such as iTunes or Google Play, find the App you chose, and download.  The first time you access the downloaded application, you will be asked to enter your FollowMyHealth credentials – login and password.

To learn more about APIs, watch this video below. If you have any questions, please ask your provider or call our office at 919-875-8150.

Thank you.

Welcome

Welcome to our Family Medicine practice! We provide continuing, comprehensive health care for individuals of all ages. And we are proud to serve as your Patient-Centered Medical Home.

Family Medical Associates of Raleigh believes that medicine is an art as well as a science. We listen to each patient to understand his or her needs. We provide care and comfort to our patients and their families to build trust relationships.

We are committed to delivering quality healthcare to the whole person — by promoting positive attitudes that maintain a healthy lifestyle and encompass the physical, psychological, social, and emotional needs of our patients.

We work in Care Teams consisting of doctors, nurse practitioners, nurses, and medical assistants in order to give you the best care we can. Our medical records staff, schedulers, and office staff are part of each Care Team. On your first visit we will encourage you to select a Primary Care Provider (PCP). Whenever possible, your appointments will be with your PCP, and if not, with another member of your Care Team.

The members of your Care Team are available to help you.
